Not able to use my ultimate ability in Apex

What happens when the bug occurs? I lose the ability use my ultimate and when i try to use it my character starts the animation and then cancels it but will randomly use it by itself
What should be happening instead? My ultimate should be used

So starting with the Chaos theory event i have been unable to use my ultimate with any of the characters after using it once in a match. Before the event it was fine. I use a ps4 controller and didn't need to use steam big picture or anything to connect it i just plugged in and go perfectly fine. I have matches where i am able to use my ult once then it never lets me use it again the rest of my play session wither I'm using my controller or switch to mouse and keyboard, no matter what character im playing. Sometimes when playing though my ult will randomly get used without me hitting the buttons to trigger it like the moment i sent out Cryptos drone it will use the EMP or sometimes it will random deploy Revenants death totem. Ive tried to switch up key binds and ive tried to switch controller schemes but you cant just remap controller buttons to ones you want. Ive tried new controller, new USB cords, with/without steam big picture and nothing has worked.
