If you intend to sneak, select a passing play with the quarter back under center (directly behind the center, not a few yards back). And as soon as you hike the ball hold the analog stick up and he will sneak.
As for shotgun plays, the only way to scramble is to 'shuffle' past the line where your qb will begin to run.
