The issue is to do with server progression not saving player data properly.
I have gotten news from @EAHelp that Respawn is investigating the issue currently and hopefully should release a fix today.
Try the following steps to try prevent this issue happening:
1. Launch Apex.
2. In the home screen, access data centre.
3. Pick another server with a low ping but different from the one you was previously in.
