Where did the issue occur? In a match
Which part of the map or menu were you in? If you don't remember the exact name, please describe the area or what you were trying to do in the menu. Capturing B on the new LTM map
What were you doing when the issue occurred? E.g. respawning a squad mate at a beacon, using an ability, a squad mate left the game. I was killing a team mate and my game lagged back to the main screen. Next game I load into my gamer tag is :"Twitch","display name": and it shows both in the main menu and in game.
Did your squad mates also experience the issue? I don't know
How many matches had you played in a row before the issue occurred? About 3. Idk I literally just got on for the first time today.
When did this happen? (dd.mm.yy hh:mm) 24.02.24. Around 10-10:15am (Pacific time)
How often does the bug occur? Rarely (0% - 9%)
How severe is your issue? Little to no impact to gameplay
What happens when the bug occurs? Nothing happens. However. I have started to stream alot more and I don't want my viewers to see something else rather then my gamer tag.
What should be happening instead? My gamer tag has been replaced. Instead of reading CheetaP155. It says twitch display name
Steps: How can we find the bug ourselves? It's my gamer tag. If there is a way to get it back to CheetaP155 both in game and in the lobby would be appreciated 🙂.

It won't let me upload the image. However my gamer tag is no longer my original gamer tag which was CheetaP155. Now my gamer tags is showing up as 

:"Twitch","display name": 

Instead of 

CheetaP155

I don't know if I was hacked or not. But I would love my old gamertag back. It means so much to me. Like I said. I just started streaming consistently for almost 3 months now so I would like your help in getting my gamertag back and hopefully fix what's going on.
