Unable to record Voice chat in Game on PS4

What happens when the bug occurs? The voice chat in game does not get recorded on the PS4. Thus resulting in some players abusing. Such as the two squad mate's listed above.
What should be happening instead? All audio, including voice chat in game should be recorded.
Steps: How can we find the bug ourselves? 1) Play the game 2) Use the Share button to record a video clip 3) Go into your library, and notice the video has no in game voice audio recorded.
The offenses of my squad members will be reported in a ticket as to not name and shame. However this issue in general is getting out of hand. Players are getting away with Racist, Homophobic, Insulting, etc.... With no valid way to record and report using the PS4.
These players are laughing about it. EA should follow behind Bethesda and permanently hardware ban such people. I'm sure most people have seen the "Bethesda bans Fallout 76 players for life after shocking in-game homophobic attack" adds. Well it worked, and it curbed the behavior of the community.
