DXGI_ERROR_DEVICE_REMOVED or DXGI_ERROR_DEVICE_HUNG

I poked around and saw a few more players having the same issue on Steam as well - Steam Link. If you can, please provide the following for us:

EA ID/Steam ID
OS installed:
OS Version
GPU:
GPU Version
Date/time issue occurred?
Summarize your bug:
What type of issue do you have? Crash - Disconnect - kicked to main menu, etc.
In which mode has this happened? MP - Campaign - if MP, which part?
Any other errors when they happen?
Crash the entire game, or just pops up the error?
Is this during a specific mission or during a cut-scene?
Can you attach a DxDiag?

Date/time issue occurred? Every time when i want to join to game single or multi
Summarize your bug: Crash game some time i can play 5min and its get crash or when i want to join to the game
