Same on Xbox. This has happened to me at least 4 times since the June update with the new matchmaking for operations. It's not necessarily from checking scoreboard but from clicking menu/start button if you remained in the same operation server after completing a match. For the most part I'm still able to check the scoreboard just fine but if try to right after dying that It gets registered as clicking the menu button (rather than holding down) and takes me to menu screen instead. And that's usually when it would take me back to previous "end of round" screen where the only option is to press menu button to quit. Why can't matchmaking for Operations be like any other game mode? Can't they just show the list of servers for each operation with # of players and what map/round they're on?
