For those of you who are having problems and no solution is working try rolling back your graphics drivers on Nvidia cards to "331.40" beta drivers. This is the only solution that has worked for me, I played the game for about 3 hours yesterday with no crashes. Before I was crashing with the below error every 1-5 minutes while in game:
DirectX function "GetDeviceRemovedReason' failed with DXGI_ERROR_DEVICE_HUNG ("The application's device failed due to badly formed commands sent by the application. This is a design-time issue that should be Investigated and fixed.").
I did a complete uninstall of my 331.65 drivers, restarted then installed the 331.40 while my internet was offline and fixed everything for me. My card EVGA 570 GTX SC
