Xbox - No NG+ options after beating game

I beat the game on Xbox, it says I have new game plus unlocked. I went the menu and tried to select it but it wouldn't work, I reset the game and when I go to the menu its not there. I beat it again and it came back, I try to select the save and it wont do anything. Anyone else have this glitch?

SUCCESS. I was maxed out on saves while trying to beat the game. That is the bug. NG+ is unable to save after beating the game if all the save slots are full. I closed and relaunched the game, went to Load Game, deleted about 50 saves, then went back to my save right before the boss, I saved a new game just to check, then beat the boss. NG+ unlocks correctly and I can play it now.

As title says. Trying to start NG+ and I just load into a black void, standing on a white square, with the basic suit and no inventory. I've tried reinstalling the game, verifying files, restarting Steam / PC, going to a previous save to complete the game again to create a new NG+ launch point. Nothing works.

Trying to launch New Game + and the game just freezes then crashes while loading. Doesn't even get into the game from the menu.
I can load into my completed save just fine, just before the boss and complete it again, then when trying to do NG+ from that new completed save it just crashes again.
Running from Steam if that makes a difference.
I've tried validating files, nothing is corrupted or needs redownload.

Similar but much worse--

This has happened to me twice in the last two days on impossible difficulty during the campaign. The first time 4 hours into my game, the second 4.5 hours.

I'm on PS5, the game will flash a red object behind Isaac and then I'm hit with a loading screen.
After load, I'm in the black void.
Please fix this. I'm so sick of having to repeat the first 6 chapters on impossible.

@Liltch That’s been happening to me but I’ve encountered other issues. The game will throw me into a black screen after the reward menu pops up, or will tell me that new game plus is ready only to show me the main menu without it on there. I’ve killed the last boss 20 times by now.

Same issue on Steam. When starting a New Game+ it freezes on a loading screen. Using ALT + F4 to close the program closes the display, but does not stop the process. It's completely hung.

@Liltch I am also having this glitch and I’ve completed the boss 6 times and it still won’t let me NG+

When selecting NG+, I was prompted to choose a completed game. I can see the completed game listed but cannot select or hit “A” (playing on Xbox).

Thinking that the game wasn't saved properly after the final boss fight, I reloaded the last save and beat the game again but the problem persists.

I also deleted a few saved games and tried the final boss fight again. Again, I wasn’t able to start NG+ after completed the game 3x.

Any suggestions?

I'm literally just stuck in a boss loop every time I beat the game I just get sent back to where I saved before the fight but I'm in a glitched menu that's the start menu but in the background is my in game screen I also can't go back to the main menu the button just doesn't work

@Liltch @BAsVAN2 @majorbrony117 @MegatonPuncher try deleting saves and then hard reseting your xbox I ended up deleting about 25 saves then I was able to access ng+
