It's a known issue.
Been happening since about Patch 7.
More with each subsequent one.
There are no plans for any more patches.
The only reliable work-around is to switch to the character who has stopped doing anything whenever you notice it, make them "hop", then assign them an action.
Not the answer you wanted, but the only one around... ☹️
