62. Dragon's Bane achievement does not unlock. I'm 100% sure I killed 10 high dragons, and Dragon Hunter (Journal > Collections > High Dragons) shows 10/10, but Dragon's Bane achievement is not unlocked in Origin.

Tranquil mage Eliza (she is researching monsters remnants), at the Skyhold library, starts to tell you about some request ("Here is a request for you" - literally), after you deal with Gray Wardens at the fort Adamant. But you actually does not receive ANY request (nor operation, nor mission) from her.
