I performed a Clean Boot and ran Dragon Age Inquisition, the problem is still happening unfortunately.

I see. If you try creating a new user profile with full admin access and try again, does it still happen?

If this has no effect (I suspect it won't but it should discount some potential issues) can you navigate to C:\Program Files (x86)\Origin Games\Dragon Age Inquisition\Update\Patch and open Packages.mft with notepad?
Did you modify this earlier to get your mods working? If so, what Version number is it set to? Does deleting all mods and changing it to the Version value to 11 let you load your saves again? And does this affect the main issue?
Creating a new user profile does not resolve the issue.
The Patch version is set to 11, and I have deleted all mods but it is still reloading itself.

Hmmm, I'd try a malware scan in case something is interacting with the file interactions in a weird way.
