Random companion damage, out of combat
I have noticed this evening that my AI controlled companions are taking odd damage spikes while exploring out of combat. It's most noticable with low HP characters, which will take 2-3 bars of damage...

I have noticed it happens a LOT in Hissing Wastes (where is also happens to random animals, causing them to go red and make the party go into 'combat posture', even if it is a little fox) but it happens everywhere... sudden out of combat massive damage to an uncontrolled character (for example in Emerald Graves just now, Cassandra had full guard and health and suddenly grunts and goes to zero guard and 50% health). Very annoying.
I am also on a PC at Normal difficulty.
Yes, it happens a lot. It's like they fall from the sky. Very strange.
I see it a lot as well. Very clumsy companions and fauna. I periodically feel like yelling, "Pick up your feet, for Pete's sake! No healing potions for you, you clumsy oaf"
Was just going to pull a High Dragon and moments before the moment of truth... bam!... my tank squalks and suddenly goes to half strength... grrrrr. At least it does not seem to happen in combat as far as I can tell.
