Random companion damage, out of combat

I have noticed this evening that my AI controlled companions are taking odd damage spikes while exploring out of combat. It's most noticable with low HP characters, which will take 2-3 bars of damage out of the blue. In 130ish hours, this is my first time experiencing this. Could this be related to the hotfix at all?

In case it makes a difference, I'm playing on Nightmare difficulty, and this is happening in the Storm Coast area. Also, I am on PC, and I have not changed any settings this evening, The only adjustment I made was to change the Origin executable properties, since the workaround is no longer needed after the hotfix, which I did install.

If you have to jump or hop down to get there, assume someone in the party is going to take damage following you. It's annoying but, there are no fall damage mods that solve the problem. Just get one off Nexus or, Google for one and, read the install instructions.
