There is a memento in The Ossuary Overlook area where you fight The Fabricator mini-boss that looks like a statuette of a dragon. Once picked up, it gives +50 Caretaker Power but does not unlock a new Memento Codex Entry. This is not fixed if reloading a previous save.
This is still bugged as of Patch 5.

Given the bug, that's actually a really good screenshot! Have you repaired the game or reinstalled the game? Do you have any previous saves you can test out or a new game you have tested?
Thanks for reaching out! I have verified game files which did not find any missing or corrupted files and have tried several other saves in the Ossuary which did not help. I have seen this particular memento reported as bugged in several collectibles guides which all mention that you still get Caretaker Power for picking it up but no codex unlocked, so I don't think saves are an issue here.
Bumping this, as the latest Patch (5) did not fix this issue.
