Problem while setting up account for EA Affiliate Program

Question

Hi! I was accepted into the affiliate program and during the process of setting up my profile I had to leave the tab open for a few hours because I had other problems. When I came back to continue when I reloaded the page I got a 500 error, after the next reload I just got a blank page with headings (on picture i add)&nbsp; which do not react to my clicks in any way. I do not know what to do
