Usually the mission starts by finding Helena (or hacking the terminal at c-sec) and killing the bosses to get a pop-up. After this you can find Helena in the new Fortuna system.

I revisited both planets but it seems I looted everything possible but Hades - Dis - Klensal is still marked ad unfinished

It's not the first time I played ME:LE but the first time I got this bug
