Hybrid control buttons have stopped working

None of those things fix the issue. I have done a hard restart. Switched to stick and 94 controls and then back to hybrid and it does not resollve the issue.
Every once in a while I hit a button during a game and it might work once, but then it stops the next time I get the puck and never works again.
Hybrid controls work fine in say practice mode, but not in online games.

Yes. Multiple controllers do not work.
I have seen this same issue in various iterations of NHL all over the internet people complaining of this same issue. Many of these complaints are on this website.
How us this a new complaint? It appears that people have been complaining about this same issue for years.

I'll keep an eye out for other reports of this. If you know of any, feel free to provide a link to them. That would help with the investigation. For now, the best I can advise is to switch to a different control scheme. Have you also tried reinstalling the game?
