It's totally random, you don't always get an upgrade in a 20k pack. Sometimes I went several packs without getting any and then suddenly in one pack I got all three upgrades for one of my peashooters. Bad luck, just keep trying.
I understand it's meant to be random, but it seems like the third upgrades have been completely taken off the table (at least for me). I'm going on dozens of packs now with no third upgrades--and I'm simply not the type to be so phenominally lucky with all my other classes to have gotten the upgrades within a few packs of opening. :P
Even still, I don't think it's totally random, the game really does seem to try and give upgrades if they're available--I've been reliably getting the first two within one or two packs of unlocking a skin.

What does the 40k pack award if you already have the non-Rank-10 variants unlocked?

When you have all non-progression characters unlocked, the 40k character pack is no longer available in the store.
What packs have you been buying; the 10k packs provide upgrades too sometimes.
