Have you tried to reset 3D properties globally from nVidia Control Panel?
Reinstlal your nVidia drivers and tick "Clean Install".
Is Origin also installed on the D: drive? If not try reinstalling it on D:

Since I recongnize the files of the game so much... I'm wondering if it isn't because of the AMD CPU + nVidia GPU? I say because BF:H, I threw everything at people and nothing fixed it. One guy told me he contacted EA and they said it was a bug with AMD + nVidia...
You could give EA a shot see if they tell you the same thing (don't tell them I told you about AMD + nVidia might be a bug... that'll just murky the waters of their replies). Give them every other bit of info tho
