Locked Out

I'm playing on PS4 and was trying to climb a skyscraper, so I used ramps as a makeshift ladder.

When I got to a certain height, my character model was deleted and I couldn't fast travel anywhere.

I closed Skate., opened it, and I got black screened trying to continue.

I restarted my PS4 and it still wouldn't let me in.
