Urgost Tactical Scan Missing

Summarize your bug I accidentally opened the door he's behind from the other side of a wall and when I did it gave me the trophy for defeating all legendary bosses but it didn't give me his scan and there's no way I know of to make him spawn again or get the scan. I don't feel that it's fair nor worth the time to play through the game again all the way to that point just to get the trophy I should have in the first playthrough.
How often does the bug occur? Often (50% - 99%)
Steps: How can we find the bug ourselves? Open the door he is behind from the other side of the wall
What happens when the bug occurs? Urgost is "defeated" somehow without giving you access to his scan.
What do you expect to see? I would like for enemy scans to drop regardless of if they are killed by the player or a bug/glitch.

Urgost Fist of Rayvis and perhaps other single spawn enemies can get bugged in a state that denies the players access to their scans in the tactical guide. This prevents users from obtaining trophies.
