Yep! Except mine was a toddler disappearing, showed in the loading family screen fine but when it loaded toddler was gone. Mothers memories still had the teaching to walk talk etc but there was no name on the memory just the simoleon $ in place of a name!
I tested some families and for me it only happens when it's a new Sim - baby, toddler, whatever but it's not a Sim created by EA but me...
