This happened to me as well, I created a sim + a cat in Belladona Cove and once I moved them into a house, the cat dissapeared. Also everytime I was putting the pointer on top of that house (when you see the sims individual pictures in little squares), my sim picture appeared, and next to it a black square (as if the cat was there but not really, the cat was not showing up even in the family portrait).
Then later on the game I moved that sim (a woman) to another house and when the new house loaded, my sim had dissapeared and instead a random sim appeared (a man, with a different name)!!! The household name was still the surname of my previous sim, but the new generated sim had its own different name and surname.
Not to say, I lost all progress I made with that sim (It took me so long to convert her into a witch!) and it is nowhere to be found!
However I must say I played a bit before in another neighbourhood (Pleasantview) and had not experienced any of this, I will keep testing but I don't know if it is something happening in specific neighbourhoods or generally
