What makes you say that the original NPC who spawns, and makes an espresso or two, is an off-duty barista? Sounds like they're just a regular NPC who manages to play around with the coffee before the actual barista gets there.
Usually Hugh spawns in the middle of the cafe not even behind the espresso bar, so he 90 percent of the time manages to go off and play before accepting any orders.
The second barista has the actual red text job title when you hover over them, so either they take over and do the actual job or stand outside doing nothing.
If Hugh wasn't off duty then why is the game always summoning a second barista with the job name and having them both there at the same time and why doesn't he have the red job title?
It doesn't match other jobs like the massage therapists or mixologist where there are defined job shifts for separate sims who arrive at specific times.

Is this about the barista being replaced right away (cause I get the same with the bartender in the Windenberg lots especially) or that they hang around for a while ?

Mostly about being replaced right away. It just doesn't feel like it's working as intended, especially when you ask all three baristas the game has chosen about their working hours and having them all being identical overnight shifts starting from 11pm to 6am. That's just a big indication for me that something is wrong, when the other NPC jobs like bartender have realistic working hours.
I've seen mixologists/bartenders also leave when I arrive at bar lots, but I can mostly attribute that to arriving during a scheduled shift change – they work from 6am to 2pm, 2pm to 10pm and 10pm to 6am.
For baristas, if you arrive during the day the first guy frequently gives up the shift for the next barista who spawns in shortly after your sim.
