[OPEN] Game does not follow active sim back home from another lot

You could try clicking a Sim's icon at the bottom of the screen and transferring control. It would probably be faster, easier, and less annoying than using Manage Worlds.

I've never encountered this problem myself. I suspect it's caused by one of the mods you use but it would be hard to track down which one if the problem only happens once in a while.
