I've actually had this happen to me on both PC and console, so it's definitely not a mod issue. It's the same for me in that it doesn't happen very often, but I have encountered it, even when I've sent all sims back to home lot using occult teleport options so that it shouldn't take long for them to get there. All sims in the household will show as being in self-care mode on the home lot, yet the game stays in the lot they were visiting until I manually follow them home by going to Manage Worlds. It's an odd bug for sure.
YES this is exactly the bug that I'm talking about. all of the sims leave the lot in self care mode and you are just kinda there until you go to manage worlds and manually load their home lot. at least as far as I'm aware now it's not caused by mods or cc.
