Let me preface by stating that I have NO MODS or CC installed at all.
I've tried multiple times to order food at different restaurants and the food never comes. I get seated by the host/hostess, order from the wait staff, and then wait...and wait...and wait...for hours (and I mean double digits, NOT just 1 or 2 hours of waiting). No food has been delivered during any of my attempts at dining out.
So I was using the Japanese restaurant by Maxis - not one of the ones automatically in your library, but in the Gallery (it's in the trailer). My sims, too, would sit for HOURS (I think the longest I waited was 8 sim hours) and never get served. A few other patrons did get served at first, but then the waiters just stood at the chef stations waiting for food, while the tickets piled up, and I never saw the chefs put a single tray up after that. I was not playing with mods on. But, to be safe, I removed my mods folder completely and tried again. Same thing. So I repaired game, then "reset" by renaming my folder and letting the game create a new one, started a new game and tried again. Different restaurant (one of the diners by Maxis found in your library) and everything worked fine. So I took this same couple to the Japanese place, sat in the same seat from my other game - same thing! No food! Had my guy go to another table and order, he got food. So I'm not sure if it's just that one seat (it was on the outdoor patio, the middle of the three 2-seat tables) or something else in that particular restaurant. While I am very glad to know I found the "bug" it's too bad that something's not right with this "premade" restaurant. I actually really like the Japanese place. When I have more time I may go into build mode and see what I can see, but since I'm so new to this type of venue I'm not sure I'll be able to see what the issue is. I mean, they requested the table and ordered food just fine, no problems. They could get a waiter to compliment or insult a chef. But they just never got food.
Anyway, try a different restaurant. It's too bad that if EA has flaws in their own builds it will taint players' first impressions!

What restaurant were you using ?

Did you try re-arranging the kitchen a bit ?
Crin - I now suspect it is not the table I was at, but the food I ordered. I'm wondering if the chef is not high enough level to make the high-end experimental food on Shimuzu's (sp?) menu. My sims do not own the restaurant, so it's entirely npc-run. I don't know how the chef settings work yet, if you can specify a minimum skill level for the chef's hired? Every time I've been there it's been different NPC's in the role. I'm going to check it out tonight, see what settings I can change when I don't own the venue. But if you already know, so much the better!
Usually they should just cook it badly if they don't have the skill. At least that's what happens when you own the restaurant.
You can't really set the employees if it's NPC run.
Does it work well when you order something easier ?
I was using the Japanese one by Maxis. I tried on two separate occasions and at two different seats. Other customers were getting served so I don't know if moving the kitchen will work, but I'll try. Thanks for the tip!

Crin - just letting you know, I started a thread at http://forums.thesims.com/en_US/discussion/890135/food-never-served/p1 on this, it is mentioned a couple times in various forms on the "main" bug thread but it's hard to discuss a specific issue there. Quite a few people have the issue, and for a lot of them it's specifically Shimuzu. Some patrons get served, others never do. I personally got served in one seat but not in another. Different dishes, and of course I can't remember now the exact ones. I had no problems at all in a different restaurant (L;Attente Bistro).
Interesting note: there is a new version of Shimuzu up, the one I downloaded was created 6/9 and says it had of over 9k downloads, the one in the Gallery now says 6/10 and has about 600 downloads.... Did they fix something? I'll have to try it out.
Resolved! Please see http://forums.thesims.com/en_US/discussion/comment/14905158/#Comment_14905158. The issue is a shelf blocking the chef station in Shimuzu. In any player-made venues, there is probably something blocking the station too. The game will allow you to hang things on the wall above it, but it will not work. It looks like you can hang it higher; EA uploaded a new version of Shimuzu today that appears to work. The linked post shows a pic of what the Simmer did to fix hers, and it worked for me too.

I have experienced the exact same, though this was at one of the restaurants i've build myself.
And also, I now own my own restaurant, and a couple came and sat at one of the single tables and they never got served! They ended up leaving with 1 star, and they were very angry about the wait time. Right after, another couple came, got seated at the same table, and the same thing happened again! Is the problem the type of table?
Btw. I use the chef station which has to be placed up a wall, for the food to be taken from the middle.

It is in the Gallery under the name Daniel's Bistro. For some reason (which have been going on for a long time now) I can't find myself when searching on the gallery on thesims.com, but it works when searching in the gallery in the game. My Gallery and Origin name that I use is Kaffepigen.

I tested your restaurant and in my game, all tables are served.
I thought maybe it was the plants next to the serving windows but I had no troubles with them. Try maybe removing to see if it gets better for you.
Does the chef never cook it for you or does it get left on the tray or do the waiters pick it up but never serve ?

make sure nothing is blocking the chief station or the tables.
Which restaurant are you using ?
Which one ? There is an issue with the Japanese one.
They have fixed one part so if it's this one, you might need to download it again.
The Chez Llama ? That does seem to have a few problems.
Try moving around some things.
Different employees ? Maybe it's connected ?
