@LeahBray1992 I just had the same issue. Couldn’t remove Graduation Cap even with MCCC. I just travelled back to Foxbury Commons and the Hraduation Caps disappeared when they got there. Bought them back home and they have all gone. May be worth a try.
Sorry, obviously if they were at Britechester you need to take them back to their commons to remove the Grad Caps.

I was having the same problem with a recent grad. She'd finished a Bio degree from Britechester with an A-, but stupidly I switched to a different household before she actually finished the ceremony. I didn't even know about the picture thing, so I'm a little irked with myself about that!
Anyway, I realized that her hat wasn't coming off.
I DID manage to get it off via MCCC:
Click on your be-hatted sim and follow this path: MC Command Center -> MC Cheats -> Cheat Sim Info -> Mood/Buff Control -> Remove Buff
You'll be looking for the buff: buff_University_Graduation_Cap
Once you click that, it should immediately go away. I hope that helps!

In my game I was playing the Pleasant twins, who unfortunately went to 2 diff uni's, so I started to go to one ceremony, then the pop up flashed again, and before I realized it was for the other twin, I clicked attend, so neither of them attended their ceremony.
Apparently, you have to travel to whichever college you graduated from then travel BACK to your home lot and then the stupid cap disappears LOL I didn't like those irl either LOLOL
@maiky3777 so you traveled back to the correct uni then back to your home and it didn't take the cap off?
