I do know, both times I stayed late, and It was about promotion time.

The first time, I wasn't really that vigilant, There were small issues, like my Sim showing up in SummerClothing at work, and not changing into Work Clothes..

- But hey, I've played alle the Sims Games over the Years, and used to peculiar, small glitches in the Matrix.

- First time, however, She got the promotion AT work. It's been a long while, since I played Active careers, and didn't remember, if it's was normal or Otherwise. - Then I misdiagnosed/stayed late to get Her Work Scores a bit up again, as I remember.

Can't remember, anymore, if I had a Work Event that day, i didn't pay to much attention as said, didn't expect the Outcome of the Workday..

- At level 8, You need to deliver a baby.

I was at level 9 Going on level 10 (And got the level 10). Drop in Patients, who need Emergency Surgery, is quite normal.

Second Time, I only stayed late in hope of the Promotion, but did have a drop in Surgery Patient.

All in All:

The sure thing is both Times: Stay Late Happened.

First Problem occurred with: Stay Late AFTER Promotion occurred.

Second: Stay Late,  to GET a Promotion.

Unsure: The Work Events.

I'm not sure anymore.. The Second incident had a drop by Patient, but the first, I can't remember.

The Clothes Problem Bug sounds Similar, Yes, but it's Random Clothes, not former Work Attire. The Solo Outfit on ScreenShot resembles what She Wore, and isn't what She usually wear at Home, as shown, in the other ScreenShot.

@crinrict I sure will try. Can't promise it will be before the Weekend, though, am a bit busy these couple of days. But I'll look into it! Thanks for all Your help so far!

@crinrict  I promised my testing would be over the Weekend, but didn't had quite the amount of Time to do the testing properly, to my own satisfaction.

Better late than never though.

First of all, seems like the Problem do occur, when staying late, as You deducted after Your own testing.

Tried 10 Work Shifts without pushing to Stay Late, and Stuck at Work Mode didn't reappear.

Small Overview in the different Shifts:

Day 1:

Work Event: Deliver a Baby. Everything was as by design.

Promotion at End of Shift.

Day 2:

Work Event: Collapsed/ assess Sim. - Bugged.

- Patient moved to Exambed instead of Surgery table. Couldn't interact/click on her. (debug tried and failed) Emergency failed, and she left untreated.

Day 3:

The Mermaid Work Clothes Bug, You mentioned appeared.

Work Event: Virulent Outbreak.  Everything was as by design.

Day 4:

Mermaid Work Clothes Bug again.

Work Event: Assess collapsed Sim. Patient never showed up. Bugged.

Day 5:

Work Event: Assess collapsed Sim. Everything was as by design.

Promoted to Chief of Staff at End of Shift.

Day 6:

Mermaid Work Clothes Bug again. A lovely Evening Gown this time though..

Work Event: Deliver Baby. Everything was as by design.

Given a Raise by End of Shift.

Day 7:

Work Event: Assess collapsed Sim. Everything was as by design.

Day 8:

couldn't Join my Sim at Work because of Harvest Fest, even though The Event was closed down 3 hours prior to Work day start.

Day 9: 

Work Event: Assess collapsed Sim. Everything was as by design.

Given a Raise by End of Shift.

Day 10:

Work Event: Virulent Outbreak. Didn't show up in Work task/ Event tasks, so didn't go. Failed.

Given a Raise at End of Shift.

After this Test try, I'm rather convinced  that The Promotion Event, You mentioned, had an impact together with choosing to Staying Late action. My Sim got two promotions in this Test Game, and several Raises too. - Always at End of Shift, and not during the Work Shift, like when my Problems appeared first.

I do have The Mermaid Work Clothes malfunction bug too, clearly.

The other bugs, I've experienced, well, They could obviously happen because my Game at this time is a bit "shifty", or be Independent ones.

- They didn't ruin the Game experience for me, even though not being able to Join a Sims Active Career at Holidays is quite annoying for me.

Hope I've shed a little more light over my experiences with this bug..

@simsister  I'm sorry to correct You. It IS an issue in the Doctor Career too. I've experienced, and documented it, as far as I could.
