The menstruation cycle thing really should have been an update, already. Along with the return of facial acne from the PC version of The Sims 2 in The Sims 4.
Why do you need to see your sims bleed?
Mods for this offer very descriptive details with stained underwear and moodlets.
EA dosnt need to add this to the base becuese they have allowed modders to add this to the base game. If you want cramps, pms and stained underwear than download an adult mod. It's that simple.
