I know it is not quite the same thing, but there is an option in the game to Registering with the Ministry of Labour. In doing so you can add in a custom Job role and perhaps call it Retired Doctor ect.

I went ahead and tested it myself, and you were absolutely right - your sim can stay retired, continue receiving their pension, and still be registered with the Ministry of Labour at the same time. I’ve added a screenshot so you can see what I saw as well.
I wanted to double check before confirming anything because I was a bit worried that registering with the "Ministry of Labour" might override the retirement status or stop the pension payments.
Thankfully, that isn’t the case at all.
I can confirm: A retired Sim who’s already receiving a pension will keep getting their daily pension even after being registered with the Ministry of Labour as proven with my screenshot below.
