Sim looks different in live mode vs CAS

I just started having a very odd issue today in the Sims 4. My sim had twins that aged up to infants and I wanted to modify their appearance. I went into CAS and edited the twins however whenever I exit CAS, only 1 of the twins saves the changes. However, when I go back into CAS to re-edit the twin who's changes weren't saved, they appear in CAS exactly as I made them. I cleared my game cache and I do not have any mods or cc installed. Please help! I have attached a picture of the sim in CAS vs in Live Mode.
