Usually I barely need to change anything with my own Sims anyway, with only the rare occasions where they'll have to go through plastic surgery to make them look decent enough yet still not straying too far from their original genetics. Most others end up surprisingly normal with a few hiccups (Protip: If you're creating kids from CAS, make sure to check out what they'll look like grown up; I've learned stretching a child's genetics may end them up turning awful as teens/adults).
