Once you opt out, it's as though that sim's fame never happened. Quirks disappear, rewards stop, and while your sim can still be "normal" socially with any celebrities they were already acquainted with, meeting any new celebrities will give the whole "OMG IT'S A FAMOUS PERSON ohgodohgodohgod WOW" over-the-top fangirling reaction.
Nobody fawned over my sim at all. I had no idea she had been famous until she got a +2 angry moodlet for needing a drink. I figured the quirk was stuck since it didn’t make sense for it to be active. But it just took a little time to wear off, I guess.
