Even here in the US, 18 year olds can drink at home with parental supervision in some states (and I'm willing to bet that many parents let their adult children under 21 consume a glass of wine on occasion in every state). There are even a handful of states that let 18 year olds drink in bars.
That said, the drinks in Sims 4 are indeed all non-alcoholic except for kava.
