Yes children can be taken away if they're starving or if they have an F at school for too long, and has always been a thing since the start (this has replaced the physical appearance of the social worker in prior games, they just teleport). For grades, the game first warns you about needing to bring your grades up through a notification before this occurs, though.

Do a game repair. But, if you use mods, remove your Mods folder, first. Also, don't try to do any of this while the game is running. Go to the page on Origin where you see the covers of the game. Do a right click on the Sims 4 game and then click on Repair, then let the game do it's thing. Once it's done you can return your Mods folder and play like normal.
