When does the gender of the baby get decided?

Hmm I wonder when too! I have mc command center and it decides what gender you have at the beginning and it does not change at birth. I tried to influence it but they still had a girl. But idk about other peoples experiences

I don't know I have mcc mod and the gender and amount of children changes it seems for quite a while. One time I got my sim pregnant and the mod said she was having 3 boys and 1 girl, and then when she entered the 1 trimester it said 3 girls and 2 boys, and the second trimester it changed yet again not sure to what. So I don't know, and not sure if the mod has something to do with that.

I have come to believe that the gender, appearance and life state of the child is determined at the moment of birth. This is something I stumbled upon by accident. I had a couple conceive a child. During the pregnancy, the father was bitten and turned into a vampire. Mysteriously, the child was then also born a vampire. This leads me to believe that the game performs its genetics checks upon birth rather than at conception, which also allows for the player to eat foods and listen to music during the pregnancy to influence gender.

It was definitely a thing for Sims 3 (It was Watermelons for girls and Apples for boys and the kid's station on the radio or TV for multiples). For Sims 4 I don't know if it's ever actually been confirmed by gurus but supposedly there's food options (Strawberries for girls and I believe carrots maybe for boys) but also if your sims listen to Alternative you're more likely to get a boy and Pop more likely to get a girl. But they need to actually be listening to it not just have it on in the background. I've heard the TV thing for multiples but I'm not sure about it.

The feature to be able to influence the gender of your baby was brought into the game in May 2015 through a game patch

Sims can now influence the gender of their offspring!
Strawberries and Pop music (one, the other, or both) seem to lead to more baby girls.
Carrots and Alternative music to more baby boys.

@rosemow Is there anything that influences twins except the sim and lot traits?

Having your sim watch the kids channel can help them to have multiple births.

Since MCCC (Mod) can determine gender at time of conception I was under the impression gender was determined then. You can influence it until the moment of birth however.

I never have a failure on gender when using the carrots or the strawberries. Works every time. I don't know how many to feed the mother, so I just feed her many.
