Okay here is a thorough report of the issues I'm experiencing and my hardware specifications.
In-game symptoms:
The game stutters whenever I'm moving the camera. This issue is constant but becomes more severe on lots with more people. From my tests, the issue is most severe during parties, and the game generally feels slower in these situations. Although I haven’t monitored my FPS, it seems to drop significantly during parties and events. In general I'm sure I'm generally experiencing way lower fps than before these issues occurred.
I used to play the game on 4k resolution, it wasn't the most smooth experience, but it worked. When I would try playing on 1440p the game would run really smoothly. Now 4K feels borderline unplayable as the stuttering is so bad. 1440p is also clearly not as smooth as it used to be and it is on this resolution I've done most of my testing and where I've noticed the issues mentioned above.

"are you using an external display? If so what is the size and resolution?" - Yes. I'm using a 27" 4K (3840x2160) external monitor.
"does this happen in windowed and fullscreen mode or just one?" - Performance is better in windowed however I think that is because it sets my game to a lower resolution when I toggle window mode.
I've also tried playing with both game mode on and off, there wasn't really a difference. Perhaps a bit more lag with it off.
Playing on a new user account on my Mac doesn't improve things either.
This is brilliant testing! Have you tried hooking up to a different monitor? Maybe a TV? There was an issue a few years ago with specific 24" displays and I'm wondering whether something similar is now happening again.
If you could monitor the fps too that would be great, fps on in the cheat console.
