@rhianna0811 The .ini will only show if you have Windows set to show file extensions. It's the same file either way.
Each time you are about to play Sims 4, please first check the timestamp on options.ini again, and let me know if and when it's been modified after the last time you played. The idea here is to see if and when it's being altered, or replaced or whatever is happening, and then try to figure out what's interfering.
If the options.ini file has the correct timestamp but your in-game options are reset anyway, that's also useful information, so please let me know either way.
The time stamp always seems to be correct? I don’t know if this is something to do with my graphics card?

@rhianna0811 Then the next question is when the file resets, for example does it reset only after you restart your computer, or after you do something else specific? The problem here is that there's no good reason for the reset, especially if you've deleted the old options.ini and allowed the game to create a new one. If there's a trigger for the resets, that information would be useful, so please take some notes about what you're doing on your computer and let me know if you notice a pattern.
Since we're on a fishing expedition, please also run a malware scan. I'm not saying you have a virus, only that it's worth checking whenever a computer is behaving oddly. Download Malwarebytes (the free trial is fine) from here:

Open it, click Scan > Custom Scan > Configure Scan, check all the boxes both for all scan types and for your all your drives, and let it do its job. This could take anywhere from a few minutes to several hours, depending on the amount of data and whether the scan finds anything., so you may want to set it to run while you're doing something else. Please attach its report so I can take a look.

Each time it resets, the date/time is the time I last closed the game
i ran a malware scan and nothing showed up

@rhianna0811 Does this still happen with Norton disabled?
Other than that, I'm sorry, I don't know what else to tell you. I can ask around, but I personally am out of ideas here; there's no other reason I can think of why the options file would reset.
