I recently went in a chat with EA to try and fix this issue, the person said they had put the Elixer into my account but unfortunately when I checked it wasn't in. Apparently my account has its own separate problem which requires a specialist so EA are going to email me eventually when they have solved my case. Could be anytime as they couldn't give me a timeframe( which worries me as it could just be thrown to one side and forgotten).
They did say they have had lots if people asking them about this issue though, but unfortunately it didn't help me and I didn't get any free donuts. ((((

Reinstalling the game fixes some issues, but it's definitely not going to help with this one. The only way this problem is getting solved is if support manually adds elixir to your account, at least until a patch is rolled out.

Yep, my game crashes but it is due to my device not being the best out there, so I'm not wasting my time doing that
I finally got the 4000 elixir. EA issued it to me earlier today, but it wasn't showing up on my game so they had to do it again. The 30 donuts I was given was worth the wait.

No. They need your username, email address, and Mayhem ID.

I dunno. I think it's a device ID or something. You pull it up by clicking the little "i" button in the build menu. Then you click the Info and About, and it's the stupidly long number under your version.
I got a response today which only gave me 200 elixir. Multiple times in the email it said that it was a 200 elixir bundle. I was also given 5 donuts. It amazes me how different the responses are.

FWIW: The specialist I spoke with Saturday confirmed the amount to be awarded when reaching 9500 coins is 4000.

No it's an Origin thing.
Go to the build menu, tap the "i" in the top right corner, then tap "view" next to info, followed by "view" next to "about."
The long number sequence underneath the version number is your unique Mayhem ID.
