• Changed name is the likely reason. Try above suggestion. You can also try hitting the + sign at the bottom of the friends list in Origin and typing their name there and deleting. It won't always work but did for a few on my list.

• I can't get the hospital to start either . All docs task are done twice and everyone's had a checkup. One of the guides says Kent gets a checkup but he doesn't have one listed.
