Players disappearing during quicksim in career mode

My manager career mode seems to have a few issues. Firstly, when quicksimulating matches, the game replaces some of my newer players with other players. The players in my real lineup just disappear for the match. I've tried to download squad updates or creating new team sheets, but the problem wont go away.  Its always the same lineup that doesnt even exist anymore.

Also in one match i won 18-17 on penalties but after the match the game told me i lost 17-2 which isnt even possible on penalties. Somehow the game hasnt registered 16 scored penalties.
