There's a similar issue reported for consoles, however that issue is fixed by randomising clothes. With this issue, pressing randomise does not seem to do anything.

This save file (and in fact, the save folder it's in) has no mods or CC.

Save folders don't have mods (there's a few mods that add info there but those dort' do anything with the actual mod) , the mods folder does. Is that empty ?

  Yes, and there is nothing mods-related stored in the saves folder that this save is in.
