This is happening in my games as well and I never use mods or CC it's really frustrating because of the time it wastes in game. It's still happening in a completely vanilla game I have started to test if the latest patch had in fact fixed what it was supposed to.
Windows gaming laptop, direct X11, no mods/cc.
