Update:
More milestones with double notifications: "Stretching", "first blowout", "peed on caregiver", "lifting head"
I’ve given both infants bubble baths. One got two notifications for "first bath" and one notification for "first bubble bath", while my other infant got two notifications for "first bubble bath" and one notification for "first bath".
Got a single notification for "First laughter".

My newborns aged up to infants, and along with their first achieved milestones came double notifications.
The milestones I’ve gotten so far, divided between my two infants: "smile", "first guest" and "babble"
I’m not sure if it’s all milestones or just these, but I assume it’s all.
New save, new sims. Mods removed.

I have twins and the double message only happening with one infant. Just aged up to toddler so I'll see if the double message continues.

I have been noticing this too. Some notifications will have three in a row, others will be two or one. It seems pretty varied with play sessions. Not sure if it happens more, the longer the play session.
Playing on PC. Never any mods. Never any CC. All packs owned, except some kits.
