Hi there,
In my case, I've tried the Halloween costumes and any of my sims has had the appropiate hat with their costume.
One of my sim twins was supposed to be a villain, but he's wearing a ridiculous wool hat, and the other one was intended to be Darth Maul, but the head is not disguised.
I've never had cc or mods.
I noticed the same issue today in my game with costumes. All my sims that put on Halloween costumes were either missing the headwear or had a random one
