could you test if this workaround helps you ?
To solve this issue, disable acne in the game options, take a shower and acne should be gone.
Then turn it on again in the game options.

My teen Sim aged up with acne, and now my young adult Sim has it. I turned off acne in the pack settings, and he took a shower, but he still has acne. I even had him freshen up. I also checked in CAS to ensure he didn’t have acne under the skin details, and he doesn’t. It’s definitely a known bug, and there’s a Reddit post about it. I’ve tried all the workarounds, and it seems the only way to clear this up is to use a cheat; however, that’s not an option for me because I’m trying to earn trophies. Hopefully, this gets fixed!

I know that this has been a problem in the past with teenagers aging up into adults while having acne. I’ve experienced it in the past, but there was at least a workaround.
With the fairies, I think it’s different because they have their normal form, and their fairy form. I only use their fairy form, so the acne only shows up on their fairy form. I’m not sure if other occults are like this, I rarely play aliens, and mermaids, and I don’t use vampire’s dark form much. But I assume so.
In the past with normal Sims, you can turn acne on, and then off again in the settings, and sometimes that works. Or you can turn it off in settings, and have the Sim shower and freshen up. I turned the acne setting off, then completely exited my game, restarted it. But no methods have worked so far. I’ve also tried these in both normal and fairy form. And it’s not removable in CAS.I don’t use mods.

Currently also dealing with this issue, and i’ve even made them non occult and it’s still not working. I’ve tried everything, face mask, turning it off in the settings, restarting the game, nothing is working. I’m so frustrated because I don’t wanna enable cheats.

I have this bug too. I have tried disabling the acne setting, having the sim shower (multiple times), and even restarting the game and then repeating the process. The acne won't go away. My sim isn't an occult.

.... perhaps it is not a bug, but life.
In real you can Remain Acne scars too.
I Decided to let the setting on, it is something my sims (& me) have to live with.

Hello, I am also experiencing the same problem, and no method works to remove it.
I have tried:
- Cleaning the acne in-game
- Disabling and re-enabling acne in the pack settings
- CAS cheats (CAS.fulleditmode) and age changes
- Transforming into various occult forms
- Cloning via MCCC
- Acne solutions for young adults
Result: The acne remains, even on a clone or a random Sim created from her. The bug seems to be a corrupted skin detail.
I am playing on PC via EA.
Like Karfunkel2, I think this might be a life consequence, like acne marks that don’t go away with age, but I admit it’s frustrating, especially since I am using this character for a specific story.
