I am also having this issue. My save is modded but I have also tried this in a clean save with no mods. It happens with two out of five mermaid sims I have in my game. The "clean" mermaid sim I made with no mods or cc in a fresh save also gets it once I start putting on tattoos and layering them.
In addition to this, all mermaids lost their tails after the tattoo/B&H update. They can be reapplied but I still found it bizarre.
