I'm having the EXACT same problem and I don't use mods and CC.
I have two teens in my household. One whose father was Grim and one who wasn't. Only the teen from Grim becomes distorted. Her body shrivels up to bone and skin and her face goes up and shrinks. I'm tired of each time going to CAS just to fix her appearance.
